Active Therapy, North Vancouver – Direct Billing to Your Insurance Company
We can bill your benefits plan in real-time, straight from the reception. It will save you time and effort to submit your receipts to your insurance provider and will reduce any upfront, out-of-pocket expenses related to your treatment so you can focus on you!
How it works
To bill your extended health plan, our team requires further information from you. When booking your first appointment, please let us know in the notes section of the booking about your interest in direct billing. Alternatively, after booking your session, you can phone our reception at 604-973-1700 or email [email protected] and we will send you an authorization form. This online form requires the following:
- your name*
- your date of birth
- a policy number and identification number
* if you are covered under someone else, we require the name and date of birth of the person who is covered.
If you are unable to fill out the required information prior to your appointment, we ask that you bring your insurance information and valid government-issued photo ID with you to your session. From there, Active Therapy will take care of the rest.
In certain instances, insurance companies require a doctor’s referral. We do advise that you check in with your insurance company prior to booking to confirm whether you need one or not.
You can find out what you are covered for from your extended health care company either via phone or online. In some instances, your employer may also know your details. Because this is strictly confidential, Active Therapy Clinic does not have access to any information pertaining to your insurance limits and the scope of your coverage. We can only provide you with information regarding coverage specific to an appointment and only after we have submitted for your appointment.
Keep in mind, because of the confidential nature of insurance plans, there can be certain instances in which we are unable to submit your claim on your behalf. If there are any issues with your billing, we will inform you, collect payment and send you the receipt, so that you can submit on your own.
If your insurance company only covers a portion of your visit, we will still submit for you! Our reception team will notify you of any outstanding payments and provide you with a receipt upon payment.
We can bill to one extended health care company per visit. If you have multiple health care plans, we can provide you with a receipt for any outstanding amounts, and you can submit this to your secondary provider on your own.
Active Therapy Clinic can only submit to your insurance company on the day of your appointment. We cannot backdate appointments.
*Please be aware that Osteopathy is not regulated and Manual Osteopaths may have different coverage depending on their association they are affiliated with. Check with your benefits administrator for information about your group plan and benefits with respect to coverage for the services of a Manual Osteopath. It is important to note that osteopathy treatment can not be billed directly to insurance companies. You will be given a receipt with the practitioner’s license number to submit to your insurance provider.
